Recovery from otoplasty is generally well-tolerated, though the first several days require attentive care to protect the ears and support proper healing. Most patients are surprised by how manageable the process is when they know what to expect. Following the guidelines below closely will help you heal comfortably and achieve the best possible outcome.

What to Expect After Your Procedure

Immediately After Surgery: Upon waking from anesthesia, you will find gauze padding and protective cups secured over your ears. Do not be alarmed if your hearing feels muffled or diminished. This is entirely expected and is simply the result of the dressings. Most patients spend 30 to 60 minutes in the recovery area before being discharged to go home.

Swelling and Bruising: Swelling and bruising around the ears are a normal part of the healing process. Discomfort tends to gradually increase over the first 3 days before beginning to subside. It is not unusual to feel worse on day 4 than you did on day 2. By the end of the first week, most patients notice a meaningful improvement.

Pain: Some degree of soreness, tightness, and pressure around the ears is expected, particularly in the first 3 to 5 days. Your prescribed pain medication will help keep you comfortable during this window. After 5 days, most patients transition to Tylenol (acetaminophen) for any remaining discomfort. Ibuprofen may be used after 2 weeks, once the risk of bleeding has passed.

Suture Removal: Sutures are typically removed at your one-week post-operative visit. Depending on how healing progresses, removal may take place anywhere from 7 to 14 days after surgery. You will also be seen for a post-operative check the day after your procedure, so we can assess your early healing and answer any questions.

Compression Dressing: A compression head wrap may be provided following surgery and should be worn as much as possible for the first 3 to 6 weeks. This garment helps the ears settle into their new position, reduces swelling, and protects the surgical site during sleep and daily activity.

When to Call Our Office Immediately: Please contact us right away if you experience any of the following:

  • A fever of 101 degrees Fahrenheit or higher
  • Changes in vision or double vision
  • Redness that is spreading outward from the incision site
  • Unusual or rapidly worsening swelling, particularly around one ear
  • Active bleeding that saturates more than a 4×4 piece of gauze
  • Pus or cloudy drainage from the incision site
  • Pain that is not relieved by your prescribed pain medication

What to Avoid After Your Procedure

Protecting the ears during healing is paramount. Please observe the following restrictions throughout your recovery:

  • Do not drive for at least 24 hours after taking any prescribed narcotic pain medication, and not until you are able to turn your head comfortably without straining the incision site.
  • Do not engage in strenuous exercise, heavy lifting, or bending down to lift objects for at least 4 weeks after surgery. Elevated blood pressure and physical strain increase the risk of bleeding and can compromise healing.
  • Do not engage in any sexual activity for at least 2 weeks following surgery.
  • Do not apply heat or cold to the ears. Neither warm compresses nor ice packs should be used on or around the surgical area.
  • Do not submerge your ears or incisions in water (including pools, hot tubs, lakes, or baths) until the incisions are fully healed and free of any scabbing, which typically takes 4 to 6 weeks.
  • Do not wear earrings until you have been specifically cleared to do so by Dr. Daraei.
  • Do not use a blow dryer on the ears or incision area unless instructed otherwise.
  • Do not smoke or spend time around smokers for at least one month after surgery. Nicotine restricts blood flow and significantly impairs the healing process.
  • Do not consume alcohol during the first week after surgery, as it can raise blood pressure and increase the risk of bleeding.
  • Avoid direct sun exposure to your incisions. Once the incisions have fully healed (generally 3 to 4 weeks), you may begin applying a broad-spectrum sunblock to the area when outdoors.
  • Avoid aspirin, ibuprofen, and vitamin E supplements for 2 weeks after surgery, as these can thin the blood and increase bleeding risk.

General Post-Operative Wound Care Instructions

Hygiene

For the first 48 hours after surgery, you may wash from the chest down, taking care to keep all dressings clean and dry. After 48 hours, remove the protective cups and gauze. You may then shower and gently cleanse around the ears. Do not pull the ears forward or apply any direct pressure. Replace the protective cups or compression band over the ears after showering.

Sleeping Position

For the first week, sleep on your back with your head elevated at approximately a 45-degree angle. Placing a pillow beneath your knees can add to comfort and reduce strain. Continue wearing your protective cups over the ears while sleeping to guard against accidental pressure or impact. Change your position or get up briefly every 2 hours during the day and night to reduce the risk of blood clots.

Blood Clot Prevention

Frequently flex your feet by pointing your toes upward toward the ceiling and pushing your heels downward. This simple movement activates the calf muscles and helps promote healthy circulation during periods of rest.

Medications

Take all prescribed medications exactly as directed. Use narcotic pain medication (such as Percocet or hydrocodone) only as needed for the first 3 to 5 days, and always eat something before taking it to minimize the risk of nausea. If nausea does occur, take your prescribed anti-nausea medication approximately 20 minutes before your next dose of pain medication. Because narcotics, anesthesia, and reduced activity can cause constipation, begin taking an over-the-counter stool softener such as Colace the night of your surgery and continue while on narcotics. Patients prone to constipation may prefer Miralax or Milk of Magnesia.

If antibiotics were prescribed, they should be started 2 days prior to surgery and taken for the full prescribed course.

Diet

Clear liquids and a soft, bland diet (such as broth, apple juice, crackers, or gelatin) are recommended for the first evening after surgery. If well-tolerated, you may progress to a normal diet the following day. Stay well hydrated throughout your recovery, as adequate fluid intake supports healing and helps offset the constipating effects of pain medication.

Return to Work

Most patients with non-strenuous jobs are able to return to work within 5 to 7 days following surgery.

Otoplasty Results

Otoplasty delivers lasting, transformative results. Once the ears have fully healed and settled into their corrected position, patients enjoy a more balanced, symmetrical facial profile that looks entirely natural. The changes are also permanent.

It is important to remember that final results are not fully visible in the early weeks of recovery. Residual swelling can temporarily obscure the outcome, and the ears will continue to refine over the course of several months. Most patients see their true result emerge within 2 to 3 months, with continued subtle improvement beyond that.
