What is Open Rhinoplasty? 

Open rhinoplasty is a surgical technique that involves making an external incision on the columella, the small strip of tissue between the nostrils. This allows for enhanced access to the underlying cartilage and bone, for precise reshaping. Open rhinoplasty can correct concerns such as nasal asymmetry, a deviated septum, a drooping nasal tip, a flat nasal bridge, and dorsal humps. This procedure is beneficial for individuals seeking significant nasal refinements or those undergoing revision rhinoplasty. 

Candidates for Open Rhinoplasty

Ideal candidates for open rhinoplasty are individuals who are dissatisfied with the appearance or function of their nose. This procedure is suitable for those with concerns such as a bulbous or drooping tip, a nasal hump, wide nostrils, or breathing issues. Candidates should be in good overall health, non-smokers, and have realistic expectations regarding the procedure results.

Open Rhinoplasty Procedure 

Open rhinoplasty is performed under general anesthesia to ensure patient comfort. A small incision is made on the columella, allowing access to the nasal framework. Dr. Daraei then reshapes the bone and cartilage to achieve the desired contour and structural support. What is the difference between an open and closed rhinoplasty?

The main difference between an open and closed rhinoplasty is incisions and approach. Both open and closed rhinoplasty techniques are used to change the size and shape of the nose. In an open rhinoplasty, one incisions is made along the columella of the nose and addiitonal incisions are made inside the nostrils. This allows the skin to be elevated so the surgeon can see the cartilaginous and bony components of the nose more easily. The open rhinoplasty approach also allows for more advanced grafting and structural techniques. The columellar incision heals exceedingly well in experienced hands. A closed rhinoplasty uses incisions only on the inside of the nostrils to allow the surgeon to manipulated the cartilaginous and bony components of the nose. Certain ligaments are not divided and the closed approach is consider less invasive compared to the open approach. However, there are some limitations to the closed approach depending on the patient’s anatomy and desires. Each surgeon is different, but for Dr. Daraei, an open and transparent discussion with the patient is critical to determine the best approach and the best way to achieve the patient’s goals. Of note, all of Dr. Daraei’s revision rhinoplasties are performed via an open approach.

When will I see the results of my open rhinoplasty?

Patients notice results after any rhinoplasty immediately after the cast is removed at 7 days. However, due to the swelling and bruising, you will likely not see significant improvements for several weeks. After 2-4 weeks, you will see more obvious results, but it can take up to a year for all the residual swelling to go down, long in revision rhinoplasty cases.

How long does an open rhinoplasty usually take?

Typically, an open rhinoplasty can take anywhere from 2-3 hours. That said, it can depend on the complexity of the procedure and the patient’s anatomy and previous medical/surgical history.

When can I return to work after an open rhinoplasty?

Most patients return to work after 1 week. However, depending on the type of job, some patients elect to take more time off, particularly for labor intensive jobs. On average, most patients need 1-2 weeks off work.

What concerns can an open rhinoplasty address?

An open rhinoplasty can address everything from cosmetic concerns to functional ones. It can improve a deviated septum, problems breathing, wide nostrils, nasal tip issues, nasal bumps and humps, and a crooked nose.

Is an open rhinoplasty painful?

As you will be under general anesthesia, you won’t feel any pain whatsoever. After your surgery, you may feel some discomfort. The most common complaints after rhinoplasty surgery include a mild headache between the eyebrows and mild tip soreness. Dr. Daraei will prescribe pain medication during the pre-operative appointment to ensure you’re as comfortable as possible throughout your recovery.

Is there any visible scarring with an open rhinoplasty?

You may have some scarring on the skin between the nostrils. This is the only external scar and heals incredibly well. Once healed, the scar is very minimal and discreet.
